Postoperative Strahlentherapie der Thoraxwand beim Mammakarzinom des Mannes

Zusammenfassung

Einleitung: Wir untersuchten den Stellenwert der postoperativen Strahlentherapie der Thoraxwand bei Patienten mit einem männlichen Mammakarzinom, insbesondere in Hinblick auf die lokale Kontrolle und das Überleben. Methodik: 25 Patienten mit 26 histologisch gesicherten Karzinomen der männlichen Brust erhielten eine postoperative Strahlentherapie der Thoraxwand (THW) nach Mastektomie, bei 15 Patienten wurden die regionären Lymphabflusswege (LAW) bestrahlt. Die mediane Gesamtdosis betrug 60 Gy in 2 Gy Einzeldosis pro Tag an der THW und 46 Gy Median an den LAW. Neben der Radiotherapie wurde bei 13 Patienten eine adjuvante Hormontherapie und bei 3 Patienten eine adjuvante Chemotherapie durchgeführt. Das mediane Alter lag bei 62,2 Jahren (45,9-78,5 Jahre). Der mediane Nachbeobachtungszeitraum beträgt 15,3 Jahre (7,7-27,5 Jahre). Ergebnisse: Das Gesamtüberleben nach Strahlentherapie beträgt 28 %, das krankheitsspezifische Überleben 64 %. Das aktuarische 3-, 5- und 10-Jahres-Gesamtüberleben liegt bei 72 %, 56 % und 35 %. Die mediane Überlebenszeit lag bei 6,1 Jahren. Das aktuarische progressionsfreie Überleben beträgt 80 %, 52 % und 43 % nach 3, 5 und 10 Jahren. Die lokale Tumorkontrollrate liegt bei 92 % (24 / 26). Der positive axilläre Lymphknotenstatus (p < 0,01) und die Lokalisation des Tumors in der rechten Brust (p < 0,04) zeigten einen signifikanten Einfluss auf das Gesamtüberleben. Schlussfolgerung: Die postoperative Strahlentherapie ist ein wichtiger Bestandteil der Therapie des männlichen Mammakarzinoms zur Verbesserung der lokalen Kontrolle und des progressionsfreien Überlebens. Das Vorhandensein axillärer Lymphknotenmetastasen verschlechtert das Überleben signifikant.

Abstract

Introduction: We analysed our long-term results with postoperative radiotherapy of the chest wall in male breast cancer patients with respect to local control and survival. Methods: Twenty-five patients with 26 histological proven carcinomas of the male breast underwent postoperative radiotherapy of the chest wall with (n = 15) or without regional lymphatics after mastectomy. Additionally 13 patients received adjuvant hormones and 3 patients adjuvant chemotherapy. Median age at treatment was 62.2 years (45.9-78.5 years). Median follow-up was 15.3 years (7.7-27.5 years). Results: Overall survival after radiotherapy was 28 %, disease-specific survival was 64 %. Actuarial 3-, 5- and 10-year survival was 72 %, 56 % and 35 %. Median survival time was 6.1 years. Actuarial progression-free survival was 80 %, 52 % and 43 % after 3, 5 and 10 years, respectively. Local tumor control was 92 % (24 / 26). Survival was significantly affected by the presence of lymph node metastases (p < 0.01) and localisation of the tumor in the right breast (p < 0.04). Conclusion: Postoperative radiotherapy is an important part of the management of male breast cancer to improve local control and progression-free survival. The presence of lymph node metastases significantly impairs suvival.
