Heterotope Ossifikation nach offenem Schädel-Hirn-Trauma

Fallvorstellung

Ein 53-jähriger Mann erlitt im März 2006 ein offenes Schädel-Hirn-Trauma nach transorbitaler Pfählungsverletzung des Neurokraniums durch einen Schraubenzieher. Weitere Traumata lagen nicht vor. Nach mehrmonatiger neurochirurgischer Intensivtherapie konnte der zu diesem Zeitpunkt noch bettlägerige Patient mit milden neurologischen Residuen nach Hause entlassen werden. Im Rehabilitationsverlauf kam es zu rezidivierenden Krampfanfällen und im April 2007 zu einer plötzlichen massiven Luftnot, weshalb eine Lungenembolie und eine Beckenvenenthrombose durch eine CT ausgeschlossen wurden. In dieser CT fand sich als Zufallsbefund eine neu aufgetretene Verknöcherung des M. iliacus links; klinisch war die Beweglichkeit der linken Hüfte fast vollständig aufgehoben bei einer bestehenden Kontraktur in Adduktion und Flexion.
