Zusammenfassung
Die stereotaktische Strahlentherapie ist eine moderne schnittbildgestützte Bestrahlungstechnik,
welche unter Verwendung von externen Koordinatensystemen eine hochpräzise und nebenwirkungsarme
Bestrahlung im Zielgebiet erlaubt. Diese Behandlungsform ist für Bestrahlungen von
Hirntumoren gut etabliert und wird zunehmend auch bei extrakranialen Tumoren (Lungentumoren,
Lebertumoren, Prostatakarzinomen) eingesetzt. Der Erfolg der stereotaktischen Behandlung
hängt entscheidend von der Genauigkeit der Zielvolumendefinition ab. PET liefert wichtige
Informationen zum Tumorstoffwechsel und gewinnt als Ergänzung zu den morphologischen
bildgebenden Verfahren (CT, MRT) für die Planung der stereotaktischen Therapie immer
mehr an Bedeutung. In dem vorliegenden Artikel werden Indikationen und apparative
Voraussetzungen für die Integration der PET-Untersuchungen in die stereotaktische
Bestrahlungsplanung sowie erste Ergebnisse der PET-gestützten Radiotherapie bei Hirntumoren
und einzelnen Tumorentitäten im Bereich des Körperstammes diskutiert.
Abstract
The stereotactic image-guided radiotherapy is a modern image-guided irradiation technique
which use external coordinate systems and allows a highly selective radiation of target
volumes with only minor side effects. This treatment is established for radiotherapy
of brain tumors and is used more frequently also in extracranial tumors (e. g. lung
cancer, liver tumor, prostate cancer). The success of the stereotactic treatment depends
on the precision of the target definition. As addition to the CT and MRI, PET delivers
important information on tumour metabolism; there is a growing interest in the application
of PET imaging for planning the stereotactic radiation therapy. This presented work
reviews indications, technical requirements and applications of PET for planning radiosurgery
of brain tumours and the stereotactic body radiation therapy.
