Zusammenfassung

Bei 247 Patientinnen (medianes Alter 57 Jahre; 27-84) mit operablem Mammakarzinom wurde die prognostische Bedeutung DNA-flow-zytometrischer Untersuchungen analysiert. Ein Vergleich erfolgte mit biochemisch bestimmten Steroid-Hormon-Rezeptoren und dem Lymphknotenstatus.

42% der Tumoren waren diploid und 58% aneuploid. Einen hohen S-Phasen-Anteil (>5%; Definition mittels Critlevel-Verfahren) zeigten 32% der Tumoren. Diploide Tumoren hatten niedrigere S-Phasen-Fraktionen im Vergleich zu aneuploiden Tumoren (2,5 ± 2,2% vs. 5,5 ± 3,4%). Das rezidivfreie Überleben war sowohl bei diploiden als auch bei Tumoren mit niedrigem S-Phasen-Anteil (≤5%) besser im Vergleich zu aneuploiden Tumoren (p = 0,02) und Tumoren mit hohem S-Phasen-Anteil (p = 0,004). Es konnte eine direkte proportionale Beziehung zwischen S-Phasen-Anteil und dem medianen rezidivfreien Intervall gefunden werden. Für die Beurteilung des Gesamtüberlebens ergaben sich bisher weder für di- und aneuploide Tumoren, noch für Tumoren mit niedrigem und hohem S-Phasen-Anteil, signifikante Unterschiede. Mit Hilfe des Cox-Regressions-Modelles wird die prognostische Bedeutung von Ploidie-Status und S-Phase als neuen, von Alter, Lymphknoten- und Steroid-Hormonrezeptor-Status unabhängigen Faktoren gezeigt.

Die DNA-Flow-Zytometrie ist für den klinischen Routineeinsatz geeignet und sollte insbesondere als unabhängiger Prognosefaktor zur Planung und Stratifikation bei adjuvanten Therapiestudien eingesetzt werden.

Abstract

The prognostic significance of DNA flow cytometric examinations of 247 patients (mean age 57 years; 27-84) with operable breast cancer was analyzed. The findings were compared with biochemically determined steroid-hormone reeeptors and lymph node status.

Forty-two percent of the tumors were diploid and 58% aneuploid; 32%ofthem had a high S-phase portion (>5%; definition by the crit-level method). S-phase fractions were lower in diploid than in aneuploid tumors (2.5 ± 2.2% versus 5.5 ± 3.4%). Both with diploid tumors as well as tumors with a low S-phase fraction (<5%), recurrence-free survival was better than with aneuploid tumors (p = 0.02) and tumors with a high S-phase fraction (p = 0.0041. A direct proportional relationship was found between the S-phase fraction and the mean recurrence-free interval. As regards overall survival, no significant differences have been detected so far, either for diploid and aneuploid tumors or tumors with low and high S-phase fractions. Using the Cox regression model, the prognostic significance of ploidy status and S-phase, as new factors, independent of age, lymph node and steroid-hormone reeeptor status, is shown.

DNA flow cytometry is suitable for routine clinical use and should be used in particular as an independent prognostic factor for planning and stratification in adjuvant therapeutic studies.
