Langzeitergebnisse der Azetabuloplastik

Zusammenfassung

Die Ergebnisse einer Modifikation der Azetabuloplastik nach Albee-Lance mit gleichzeitiger Detorsionsvarisierungsosteotomie (DVO) wurden nach Wachstumsabschluß untersucht. Die Pfannendachmeßwerte lagen zu 82 bis 93 % im Bereich der Norm oder der leichtesten Dysplasiestufe unseres Klassifizierungssystems, während die Meßwerte des Hüftkopfes und Schenkelhalses einen höher pathologischen Anteil hatten. Diese Tatsache wird auf vorausgehende Hüftkopfnekrosen, ungenügende operative Einstellungen in Einzelfällen, vor allem aber auf die oft starke Revalgisierung nach der DVO zurückgeführt. Die DVO wird daher schon seit mehreren Jahren im Regelfall nicht mehr verwandt.

Abstract

The results of a modified acetabuloplasty after Albee-Lance performed simultaneously with a derotation-varusosteotomy (DVO) are investigated after cessation of growth. Measurements of the acetabulum have been normal or slightly pathologic after our classification system in 82-93 %, while the values of the femoral head and neck showed a higher pathologic percentage. This is due to avascular necrosis after conservative or operative treatment, insufficient open reductions in a smaller number, but in the most cases caused by increased revalgisation after DVO. We have therefore performed acetabuloplasty since serveral years without DVO in the most cases, so far with good results.
