Stenosierende Ureteritis und Faktor-XIII-Mangel bei anaphylaktoider Purpura

Abstract

A case report of a 6-year-old boy is presented. The patient suffered from a severe Schönlein-Henoch purpura. It was demonstrated that ureteral stenosis develops during the clinical course of this disease. This complication has to be considered in anaphylactoid purpura, since it is usually self-limiting and does not require surgical intervention. This confirms once again the necessity to look for a decrease of factor XIII activity in these patients and of the value of substituting this compound if there are severe abdominal complaints. The theoretical background of this therapeutical intervention is discussed.

Zusammenfassung

Am Beispiel eines 6jährigen Jungen mit schwerem Verlauf einer anaphylaktoiden Purpura wird gezeigt, daß sich eine stenosierende Ureteritis erst während des Krankheitsverlaufes ausbildet. Damit soll der Beweis geführt werden, daß es sich um eine krankheitsspezifische Komplikation handelt, die der Kinderchirurg kennen muß, da sie in der Regel kein Eingreifen erforderlich macht, weil sie reversibel ist. Zusätzlich wird der Wert einer Faktor-XIII-Bestimmung und gegebenenfalls Substitution bei schwerer abdomineller Symptomatik erläutert und erneut diskutiert.
