Gastrointestinale Komplikationen bei Kindern mit hämolytischurämischem Syndrom

Abstract

Eight children out of 106 with haemolytic-uraemic syndrome (HUS) had marked gastrointestinal complications requiring close cooperation between the paediatrician and paediatric surgeon. These children showed signs of ileus and peritonitis. Six children had these complications in the prodromal stage, whereas the other two children showed them in the anuria stage. The complications were closely connected with multiple erosions of the mucosa and ulcerations of the intestinal wall which resulted in perforation of the intestinal wall in two cases. In two further cases, intestinal invagination was seen which was limited toward the small intestine. All operated children died. The authors are of the opinion that prognosis in HUS with severe gastrointestinal complications is very infaust.

Zusammenfassung

In einem Krankengut von 106 Fällen mit hämolytischurämischem Syndrom (HUS) wird über 8 Kinder mit ausgeprägten gastrointestinalen Komplikationen berichtet, die eine enge Zusammenarbeit des Pädiaters mit den Kinderchirurgen benötigten. In den besprochenen Fällen hat man Ileus- und Peritonitiserscheinungen festgestellt. Bei 6 Kindern wurden die Komplikationen im Prodromal-, bei zwei weiteren im Anuriestadium beobachtet. Die Komplikationen haben einen engen Zusammenhang mit multiplen Schleimhauterosionen und Darmwandulzerationen, die in zwei Fällen zur Darmwandperforation führten. In zwei weiteren Fällen hat man eine zum Dünndarm begrenzte Darmwandinvagination festgestellt. Alle operierten Kinder sind ad exitum gekommen. Den Autoren nach, ist bei HUS mit schwerwiegenden gastrointestinalen Komplikationen die Prognose sehr ernst.
