Abstract
Treatment and course of synovial sarcoma in 12 children are reported by means of a multicentre study. 5 patients were treated in our own centre in Frankfurt (M) (West-Germany). The tumour starts to grow primarily from mesodermal cells and grows aggressively and with local infiltration. Nearly all the children were older than 8 years; symptoms were not at all characteristic and routine diagnostic procedures failed in 50%. The site of predilection is in the soft tissue of the lower extremities. No tumour capsule is formed, and this fact causes a local recurrence rate of about 60%. Hence, it is mandatory to perform not only extensive local surgery but also radical excision of the surrounding tissue and of the enlarged lymph nodes. Postoperatively all patients are subjected to cytostatic treatment; in selected cases, depending on clinical staging, radiotherapy is applied. Patients are followed up for up to two and a half years; so far, the results of our treatment concept seem to be promising.
Zusammenfassung
Es wird in einer Multizenterstudie über die Therapie und den Verlauf von insgesamt 12 Patienten mit einem synovialen Sarkom berichtet; in dieser Serie sind 5 eigene Patienten aus unserem Zentrum in Frankfurt/Westdeutschland eingeschlossen. Das synoviale Sarkom im Kindesalter nimmt seinen Ausgang vom Mesoderm und wächst aggressiv und lokal infiltrativ. Fast alle Kinder waren älter als 8 Jahre; die Symptomatik ist ausgesprochen uncharakteristisch, und auch die diagnostischen Verfahren versagen anfangs in 50%. Die Prädilektionsstelle des Tumors ist das Weichteilgewebe der unteren Extremitäten. Eine Tumorkapsel kann nicht gefunden werden, und dies führt zu der hohen lokalen Rezidivrate von ca. 60%. Hierdurch wird das weitere chirurgische Vorgehen bestimmt; Es muß nicht nur weit lokal, sondern das umgebende Tumorgewebe radikal exstirpiert werden, inklusive der vergrößerten Lymphknoten im Abflußgebiet. Alle Patienten erhalten postoperativ eine zytostatische Therapie; ausgewählte Fälle werden entsprechend ihrem klinischen Stadium radiologisch nachbehandelt. Die bisherigen Folgen im Zeitraum bis zu 2 1/2 Jahren lassen das bisherige Behandlungskonzept erfolgreich erscheinen.
