Abstract
Myofascial pain syndromes are among the most common sources of painful complaints.
Since objective signs of this disorder are often unspecific the pathophysiology of
these pain states is hard to interpret.
From the point of view of a physiologist three possible explanations are given: a)
tonic motor reflexes, b) tonic vascular reflexes and c) faulty localizations of the
pain by the central nervous system. It is suggested to test these hypotheses either
by demonstrating an abnormal tonic motor activity or abnormal vascular reactions.
The first can be done with improved methods of electromyography, the second e.g. with
thermographic methods. If no local changes are found in painful areas the hypothesis
c) remains the most likely one.
Zusammenfassung
Der Formenkreis des ,,Weichteilrheumatismus" gehört zu den häufigsten Schmerzursachen.
Da objektivierbare klinische Parameter dieser Erkrankung häufig unspezifisch sind,
ist die Pathophysiologie dieser Schmerzsyndrome schwer zu verstehen.
Unter dem Blickwinkel des Physiologen lassen sich drei mögliche Erklärungsmodelle
angeben:
(a) tonische motorische schmerzerzeugende Reflexe,
b) tonische vaskuläre schmerzerzeugende Reflexe und
(c) Fehlinterpretation und -lokalisation von nozizeptivem Input im Zentralnervensystem
Es wird vorgeschlagen, diese Hypothesen zu testen, indem entweder eine tonische anormale
motorische Aktivität nachgewiesen wird oder anormale vaskuläre Reaktionen. Ersteres
ließe sich mit verbesserten Methoden der Elektromyographie erreichen, letzteres z.
B. mit thermographischen Methoden. Sollten sich keine lokalen Veränderungen in schmerzhaften
Körperregionen finden, bleibt die Hypothese (c) die wahrscheinlichste Erklärung.
