Synovektomie des Kniegelenkes bei c.P. - Indikation

Abstract

Early synovectomy must be considered as imperative, since it probably yields the long-term results. Basing on the best long-term results re-examnined by us, late synovectomy occupies a high rank in our treatment scale. 75 to 80% of the long-term improvements, especially with regard to pain and swelling, and less pronounced with regard to function and deformation, show that the knee joint synovectomy operation remains a justified measure. When deciding on the indication, it should be borne in mind that in patients on permanent cortisone medication, the latter can be reduced or completely avoided in 80% of the cases if synovectomy is performed. In any case, it is probable that the inflammatory activity of the overall disease pattern is reduced for several years as a result of synovectomy.
We believe that this fact should be taken into consideration when deciding on the measures indicated by the individual disease pattern.

Zusammenfassung

Die Frühsynovektomie ist mit Nachdruck zu fordern; sie läßt die besten Langzeitergebnisse erwarten. Die Spätsynovektomie hat aufgrund der überprüften Langzeitergebnisse einen hohen Stellenwert in unserer Therapieskala. 75 bis 80% der langzeitigen Besserungen, insbesondere in bezug auf Schmerzen und Schwellung, geringer in bezug auf Funktion und Deformierung, lassen die Indikation zur Kniegelenkssynovektomie nach wie vor berechtigt erscheinen. Bei der Indikation soll beachtet werden, daß bei Patienten mit Cortisondauermedikation diese in 80% der Fälle vermindert oder vermieden werden kann. Eine mehrjährige Senkung der entzündlichen Aktivität des Gesamtkrankheitsbildes ist wahrscheinlich.
Diese Erkenntnis sollte bei unseren indikatorischen Überlegungen berücksichtigt werden.
