Early aneurysm Operation and vasospasm: Intracranial Doppler findings

Summary

Modern ultrasonic Doppler Systems enable us for the first time to carry out intraoperative and pre- and postoperative investigations of the circle of Willis. The application of Doppler in acute aneurysm surgery provided the following Information: During the first 3 days following subarachnoid bleeding from a cerebral aneurysm, no signs of vasospastic reaction were found by Doppler sonography. Thereafter, 90% of all patients operated on in the acute stage demonstrated localized acceleration as a sign of vasospasm, despite prophylactic nimodipine treatment. Thirty-eight percent of the patients developed delayed ischemic deficits, which, however, were only transient. These results may indicate that early surgery and nimodipine Prophylaxis may not prevent vasospasm but do reduce its consequences.

Zusammenfassung

Mit modernen Doppler-Ultraschallverfahren ist es erstmals möglich, sowohl intraoperativ als auch transkraniell, prä- und postoperativ den Circulus arteriosus Willisi zu untersuchen. Wir haben die Methoden bei frühoperierten Patienten mit zerebralen Aneurysmen eingesetzt und folgende Ergebnisse gefunden: Während der ersten drei Tage nach einer Subarachnoidalblutung aus einem rupturierten Aneurysma ergaben die eigenen Doppleruntersuchungen keine Hinweise auf eine lokale Gefäßeinengung (Vasospasmus). Danach konnte bei 90% aller innerhalb dieses Zeitraumes operierten Patienten ein mehr oder weniger ausgeprägter Vasospasmus festgestellt werden. 38% aller Patienten entwickelten unter Nimodipin-Prophylaxe ein allerdings reversibles sekundäres ischämisches Defizit.

Die Ergebnisse sprechen dafür, daß die Frühoperation und Nimodipin-Prophylaxe bei rupturierten zerebralen Aneurysmen zwar nicht die Spasmusinzidenz, aber deren Folgen günstig beeinflußt.
