Zusammenfassung

Eine Patientin entwickelte nach radikaler Zystektomie und Anlage eines kontinenten Stomas im Nabel (nach gynäkologischen Eingriffen und präoperativer Bestrahlung mit einer Dosis von 40 Gy) eine ischämische Ureternekrose links mit Stenose. Nach 6 Monaten wurde mittels bilateraler Lumbotomie eine Uretero-Pyelostomie durchgeführt. Diese Maßnahme bot 2 Vorteile: Vermeiden einer Nephrektomie sowie Umgehen einer Laparotomie mit möglicher Beeinträchtigung der Funktion der kontinenten Harnableitung.

Abstract

A female patient who had undergone radical cystectomy and continent urinary diversion with a stoma in the umbilicus (after gynecological intervention and 40 Gy radiotherapy) presented with a stenosis due to ischemia of the left ureter. After 6 months a ureteropyelostomy was formed via a bilateral lumbotomy. This approach avoided nephrectomy or laparotomy, which could have compromised the continence mechanism of the pouch.
