Zusammenfassung
Wir berichten über einen gut dokumentierten Fall einer emphysematösen Pyelonephritis,
die als seltene, aggressive und lebensbedrohliche Form der chronischen Pyelonephritis
anzusehen ist. Unter Einbeziehung der Erkrankung in die Differentialdiagnose bei unklaren
fieberhaften Erkrankungen beim Diabetiker, ist die Diagnose durch bildgebende Verfahren
(Sonographie, Computertomographie) zu sichern. Eine invasive Diagnostik (retrograde
Pyelographie, Arteriographie) ist meist nicht erforderlich. Therapeutisch ist das
operative Vorgehen unter Antibiotikaschutz und konsequenter Behandlung des Diabetes
mellitus entscheidend für die Überlebenschance.
Abstract
We describe a well documented case of emphysematous pyelonephritis. This must be considered
as a rare, aggressive and life-threatening form of pyelonephritis. After inclusion
of this condition in the differential diagnosis in febrile illnesses in diabetic patients,
the diagnosis can be confirmed by imaging (sonography, computerized tomography). Invasive
methods of diagnosis (retrograde pyelography, arteriography) are usually unnecessary.
Therapeutically, operative procedures under antibiotic protection and treatment of
the diabetes mellitus are major factors for increasing chances of survival.
