Die mikrochirurgische penile Revaskularisation - Einsatz freier Veneninterponate

Zusammenfassung

Die penile Revaskularisation nach Hauri stellt eine erfolgreiche kausale Therapie der arteriell bedingten erektilen Impotenz dar. Die beschriebenen Mo-difikationen im Sinne einer atraumatisch-mikrochirurgischen Technik unter durchgängiger Verwendung eines Operationsmikroskopes, von dünnerem Nahtmaterial und von Veneninterponaten aus der oberflächlichen Penisvene oder der V. epigastrica erweitern das operative Spektrum auf bisher aussichtslose Fälle. Dies wird anhand von Fallbeispielen erläutert.

Abstract

Penile revascularization (Hauri) is a successful operative therapy of arterial erectile impotence. We describe modifications i. e. microsurgical techniques and materials using an operative microscope, and - in cases with a short epigastric artery - the interposition of free venous grafts harvested from the superficial penile artery or the epigastric vein. Thereby seemingly hopeless cases can be operated on, and intraoperative complications can be delt with effectively as is illustrated by three case reports.
