Urethroperineale Fistel nach Rektumamputation: Verschluß durch Grazilis-Muskellappenplastik

Zusammenfassung

Urethro-perineale Fisteln sind eine seltene Komplikation der abdomino-perinealen Rektumamputation. Nach Ausschluß eines Tumorrezidivs ist der Fistelverschluß durch eine Sakralhöhlenplombe zu sichern. Die Musculus-gracilis-Lappenplastik heilt aufgrund der guten Durchblutung auch in dem stets infizierten, urinbenetzten Gebiet ein. Seine Anwendung bei einem zweimal erfolglos operierten Patienten wird beschrieben.

Abstract

Uretro-perineal fistula is a complication of abdomino-perineal rectumamputation. Once a local recurrence of the rectal cancer has been ruled out, perineal closure of the fistula is performed. The sacral cavity ought to be pluged with healthy tissue, either greater omentum or gracilis-muscle. Instead of omentum, which is difficult to obtain or may be missing due to preceding abdominal surgery, gracilis muscle is an alternative. Its application is shown in a patient after two unsuccessful attempts of closing the urethro-perineal fistula.
