Abstract
A very unusual clinical case of histiocytosis X in a young girl with no unusual medical
history, is presented. The patient presented with chronic suppuration of the groin
and complex anal fistulae and multiple recurrent abscesses, despite surgical treatment
(abscess drainage, terminal colostomy). Intestinal and immunologic evaluation were
normal. Only histology made the diagnosis. Chemotherapy (vinblastine) resulted in
healing and the colostomy was closed.
To our knowledge, this is the first case of histiocystosis X revealed by a chronic
perianal suppuration in a child.
Zusammenfassung
Der ungewöhnliche Fall eines 13jährigen Mädchens mit Histiozytose X wird vorgestellt.
Bei dem Mädchen fand sich eine chronische Infiltration und Verhärtung in der Inguinalgegend
und komplexe Analfisteln mit rezidivierenden Abszedierungen trotz Abszeßdrainage und
Kolostomie. Untersuchungen des Magen-Darm-Traktes und des Immunstatus waren unauffällig.
Die Diagnose konnte erst histologisch gestellt werden. Es wurde eine Chemotherapie
mit Vinblastin durchgeführt, die zur Heilung führte, so daß die Kolostomie geschlossen
werden konnte. Dies ist der erste Fall einer durch chronische perianale Abszedierung
sich manifestierende Histiozytose X.
