Acute and Chronic Sigmoid Volvulus in Childhood: Report of Three Cases

Abstract
Sigmoid volvulus (SV), a rare disorder in childhood, was diagnosed in three young girls. In two patients, the volvulus was successfully reduced by barium enema. In one of them, the symptoms recurred after three months and as elective operation, a long redundant sigmoid loop was resected. In the third patient, SV was acute and hydrostatic reduction failed.
Surgical management consisted of derotation and sigmoidopexy. Postoperative investigations showed that the causal factor was Hirschsprung's disease of the rectosigmoid. After Swenson's abdominal-perineal pull-through was performed, the patient remained symptom free. The underlying disorder that predisposes to SV in infants and children is a congenital anatomic or functional defect of the colon. Hirschsprung's disease should therefore be systematically ruled out.
Zusammenfassung
Der im Kindesalter selten vorkommende Sigmavolvulus (SV) wird anhand von drei Fällen vorgestellt. Bei zwei Patientinnen konnte der Volvulus mittels eines Kontrastmitteleinlaufs behoben werden, erforderte jedoch in einem Fall wegen Rezidivs drei Monate später die Exstirpation einer weit ausladenden Sigmaschlinge. Im dritten Fall von akutem SV blieb die hydrostatische Behandlung erfolglos.
Der operative Eingriff bestand aus der Derotation gefolgt von einer Sigmoidopexie. Es wurde anschließend ein auf den Enddarmbereich beschränkter Morbus Hirschsprung nachgewiesen. Nach abdominoperinealem Durchzug nach Swenson war der klinische Verlauf unauffällig. Dem SV liegt beim Kind oft eine angeborene anatomische oder funktionelle Störung des Dickdarms zugrunde. Daher wird die systematische Suche nach einem Morbus Hirschsprung empfohlen.
