Abstract
The diagnosis and operative treatment of congenital cervical fistulae and cysts are in many cases difficult, especially abnormalities of the first branchial cleft and of the sinus cervicalis.
In our hospital 90 congenital cervical fistulae and cysts were treated surgically between April 1972 and August 1979, excluding cysts of the thyroid gland, lymphangiomas etc. Four of these children with a median cyst and two children with fistulae of the first branchial cleft had recurrences due to partial obliteration and fibrosis of the ducts following inflammation. Zusammenfassung
Die kongenitalen Fisteln und Zysten am Hals können diagnostisch und operativ gelegentlich große Schwierigkeiten bereiten. Das trifft insbesondere für die Fisteln des 1. Kiemenganges und des Sinus cervicalis zu. Mit Rezidiven ist daher zu rechnen. Von 1972 bis 1979 sind in der Kinderchirurgischen Abteilung der Freien Universität Berlin im Klinikum Steglitz 90 angeborene Halszysten bzw. Fisteln operativ behandelt worden. Zysten anderer Genese sind nicht berücksichtigt worden. Bei 4 Kindern mit einer medialen Halszyste traten Rezidive auf. Die Gründe dafür waren insbesondere eine partielle Obliteration der Fistelgänge durch entzündliche Veränderungen und atypische Fistelverläufe. Bei den medianen Halsfisteln ist besonders auf die Resektion des Os hyoideum mit dem Fistelanteil zu achten. Wegen der drohenden Infektion sollten alle kongenitalen Fisteln und Zysten am Hals möglichst frühzeitig operiert werden.
