Kombinierte koronare Angioplastie und Aortenklappenvalvulotomie

Zusammenfassung

Bei einer 74jährigen Patientin mit kalzifizierter Aortenklappenstenose und koronarer Herzkrankheit wurde wegen therapierefraktärer Angina pectoris eine perkutane koronare Angioplastie in Kombination mit einer Valvulotomie der Aortenklappenstenose während des gleichen Eingriffs vorgenommen. Das komplikationslos durchgeführte Verfahren führte zu einem guten angiographischen, hämodynamischen und klinischen Ergebnis. Bei älteren Patienten mit erhöhtem operativem Risiko ist die Kombination von Valvulotomie und koronarer Angioplastie als palliative Maßnahme empfehlenswert.

Abstract

Because of treatment-resistant angina, percutaneous coronary angioplasty was performed at the same time as aortic valvotomy in a 74-year-old woman with calcific aortic stenosis and coronary heart disease. There were no complications and good angiographic, haemodynamic and clinical results were obtained. It is concluded that combined valvotomy and coronary angioplasty can be recommended as palliative treatment in elderly patients in whom operation would carry an increased risk.
