Zusammenfassung
Bei einer 60jährigen Frau mit langjährigen rheumatischen Beschwerden wurde anläßlich einer Routineuntersuchung im Differentialblutbild eine Granulozytopenie (1 % Stabkernige, 3 % Segmentkernige) bei normaler Leukozytenzahl (4200/µl) festgestellt. Als Ursache fand sich eine lymphoproliferative Erkrankung der granulierten Lymphozyten, die nach dem vorherrschenden Phänotyp auch als chronische Tγ-Lymphozytose bezeichnet wird. Die Diagnose konnte bei gering ausgeprägter Lymphozytose im Blut und Knochenmark erst durch den immunzytochemischen und molekulargenetischen Nachweis einer klonalen Expansion der Tγ-Lymphozyten gesichert werden. Im weiteren Verlauf nahm die Granulozytopenie zu, und es traten rezidivierende Infektionen auf. Durch eine Behandlung mit Ciclosporin und Cortison kam es zur Remission der Granulozytopenie.
Abstract
Routine examination of a 60-year-old woman with long-lasting rheumatic complaints revealed a granulocytopenia in the differential blood count (1 % stab cells, 3 % segmented) with a normal total leucocyte count (4,200/µl). It was caused by a lymphoproliferative disease of the granular lymphocytes, which may also be termed Tγ lymphocytosis because of the predominant phaenotype. As the lymphocytosis in blood and bone marrow was of minor degree, the diagnosis was confirmed only by immunocytochemical and molecular-genetic demonstration of clonal expansion of the Tγ lymphocytes. The granulocytopenia gradually became more marked and recurrent infections were noted. Treatment with ciclosporin and cortisone brought about remission of the granulocytopenia.
