Zusammenfassung
41 Patienten mit Granulozytopenie (< 500 × 106/l) und antibiotikaresistenten Infektionen erhielten 222 Granulozytentransfusionen, davon 212 durch Filter-leukapherese gewonnen, mit durchschnittlich 2,1 × 1010 Granulozyten. Die Infektion besserte sich bei 25 Episoden, und 18 Patienten überlebten den 30. Tag. 18 Episoden blieben unbeeinflußt, nur zwei Patienten überlebten den 30. Tag. Die Prognose wurde entscheidend beeinflußt von Ausmaß und Dauer der Knochenmarksinsuffizienz, Art und Ausdehnung der Infektion, aber auch von Risikofaktoren wie Alter (> 60 Jahre) und Thrombozytopenie (< 20 × 109/l) sowie von Leber- oder Niereninsuffizienz. Durch die Berücksichtigung klarer Richtlinien können nutzlose Transfusionen vermieden werden.
Abstract
Granulocyte transfusions (n = 222) were administered to 41 patients with granulocytopenia (< 500 × 106/l) and antibiotics-resistant infections. The average granulocyte content per transfusion was 2.1 × 1010, 212 transfusions had been obtained by filter leucapheresis. Infections improved in 25 episodes, and 18 patients survived the 30th day. There was no influence in 18 episodes and only 2 patients survived beyond the 30th day. Extent and duration of bone marrow insufficiency, and type and grade of infection, clearly influenced prognosis. Further prognostically relevant risk factors were age (> 60 years) and platelet deficiency (< 20 × 109/l), as well as liver and renal insufficiency. Adherence to precise guidelines may prevent pointless transfusions.
