Abstract
We hereby report the second case of antenatally diagnosed "neurenteric cyst" in the
literature. The patient we describe presented as a neonatal respiratory distress syndrome
(NRDS) which did not respond to a vigorous resuscitation. Anomaly of the antenatal
ultrasonography and of the initial chest x-ray added to the nonresponse to therapy,
led us to an urgent thoracic scan and to a lifesaving thoracotomy.
We think that every posterior mediastinal cystic mass, with or without vertebral malformations,
diagnosed antenatally, should raise the possibility of neurenteric cyst. The perinatal
management of the newborn to be, will thus be simplified. This will lead to a better
outcome.
Zusammenfassung
Es wird der zweite Fall einer pränatal diagnostizierten. neurenterischen Zyste vorgestellt.
Bei dem Kind bestand ein angeborenes Respiratory-Distress-Syndrom (RDS), das auf intensive
Wiederbelebung nicht ansprach. Da bereits der pränatale Ultraschall auf eine Fehlbildung
hinwies, die bei der post partum durchgeführten Röntgenaufnahme bestätigt wurde, wurde
eine Notthorakotomie und lebensrettende Operation durchgeführt. Es fand sich eine
neurenterische Zyste.
Die Autoren sind daher der Ansicht, daß im posterioren Mediastinum gelegene Zysten
mit oder ohne Wirbelfehlbildungen an die Diagnose einer neurenterischen Zyste denken
lassen sollten. Das perinatale und postnatale Management kann in dieser Hinsicht geplant
und die Erfolgsaussichten gerade bei Notfällen verbessert werden.
