Clinical and Manometric Assessment of Anorectal Function After Martin's Operation

Summary
Anorectal function after Martin's operation for total colonic aganglionosis was assessed in five patients by clinical and manometric studies.
One year after operation the frequency of bowel movements decreased to three times a day, and the stools became semi-formed. Anorectal manometry showed decreasing anal resting pressure and anorectal pressure difference one month after the operation. However, these values became gradually normal. Rectal compliance increased as bowel function improved clinically.
These results show that increasing rectal compliance and normalisation of stools by adequate absorption of intestinal fluid are important factors in the improvement of faecal continence after Martin's operation.
Zusammenfassung
Nach Durchführung einer ileokolischen Seit-zu-Seit-Anastomose nach Martin bei totaler Kolonaganglionose wurde bei 5 Patienten klinisch und manometrisch die anorektale Kontinenzfunktion überprüft. Ein Jahr nach der Operation war die Stuhlfrequenz auf 3 pro Tag zurückgegangen und die Fäzes waren teilweise geformt. Die anorektale Manometrie zeigte einen Monat nach der Operation noch herabgesetzte anorektale Ruhedrucke, die sich jedoch nach und nach normalisierten. Mit zunehmendem Abstand zur Operation nahm die Compliance zu, und die Darmfunktion erholte sich. Die Ergebnisse zeigen, daß die zunehmende rektale Compliance und die Normalisierung der Stuhlkonsistenz durch eine adäquate Reabsorption der intestinalen Flüssigkeit im Rahmen des Martinschen Operationsverfahren bewirkt wird.
