Die Behandlung des anterioren Knieschmerzes und einer Instabilität des Streckapparates ist schwierig und keine Methode ist für alle Patienten geeignet. Patienten mit patellofemoralen Schmerzen benötigen eine gründliche klinische Untersuchung und eine genaue Anamnese. Die Kenntnis möglicher Verletzungsmechanismen sowie die gründliche Untersuchung der die Patella umgebenden Weichteile ermöglicht die Eingrenzung der Ursachen für den anterioren Knieschmerz oder einer patellofemoralen Instabilität. Neben den a.–p. Standardröntgenaufnahmen des Kniegelenkes im Stehen wird eine Patellatangentialaufnahme in 30 – 45° sowie ein streng seitliche Aufnahme benötigt. Eine konservative Therapie ist in vielen Fällen erfolgreich und sollte vor jeder operativen Maßnahme ausgeschöpft werden. Die konservative Therapie beinhaltet die Krankengymnastik zum Auftrainieren des M. vastus medialis, Dehnen des M. quadriceps bei Verkürzung, propiozeptives Training, Kräftigung der Hüftaußenrotatoren, Medikamente (Antiphlogistika), physikalische Therapie (Kryotherapie, Ultraschall, Iontophorese usw.) und Orthesen (Patellabandagen, Taping).

Bei notwendiger Operation ist ein laterales Release für die pathologisch rotierte Patella (Patella Tilt) geeignet. Manchmal sind schmerzhafte Vernarbungen, Retinakula, Neurome oder Plicae zu entfernen. Die Wiederherstellung des proximalen Patellaalignments kann durch arthroskopische und/oder offene Zugänge erreicht werden. Symptomatische Knorpelläsionen und ausgedehnte Instabilitäten des Streckapparates erfordern häufig distale Eingriffe, z. B. in Form einer medialen oder anteromedialen Tuberositas-tibia-Transposition.

Die aktuelle Therapie des patellofemoralen Schmerzes schließt eine genaue Anamnese und Diagnose, eine konservative Therapie unter Einschluss der gesamten Bewegungskette, die Wiederherstellung der Patellabalance, die minimalchirurgische Intervention und eine genaue Indikation für die jeweilige Operationstechnik ein.
