[18F]FDG PET/CT for assessing inguinal lymph nodes in patients with penile cancer – correlation with histopathology after inguinal lymphadenectomy

Summary

Aim: Accurate staging of penile cancer requires invasive methods such as sentinel node biopsy or lymphadenectomy (LAD). We assessed the value of [18F]FDG PET/CT for non-invasive nodal staging in penile cancer (PC) patients before inguinal LAD.

Patients and methods: 41 consecutive patients with PC (stage pT1 or higher, cN0) received [18F]FDG PET/CT before undergoing bilateral modified or radical inguinal staging LAD. Lymph nodes with a visually increased [18F]FDG uptake were classified as suspicious of lymph node metastases (LNM). Standardized uptake value (SUV) of suspicious inguinal lymph nodes was determined. Results of [18F]FDG PET/CT were correlated with histopathology.

Results: In total 623 lymph nodes were resected, in 10 patients LNM were histologically confirmed (14/623 lymph nodes). In patient-based analysis [18F]FDG PET/CT showed a sensitivity and specificity of 80% and 68 %, respectively, a positive predictive value (PPV) of 44 % and a negative predictive value (NPV) of 91 %. In the groin-based analysis, [18F]FDG PET/CT had a sensitivity of 69 %, a specificity of 77 %, a PPV of 36 % and a NPV of 93 %. There was no significant difference in SUVmean and SUVmax between true positive and false positive lymph nodes (p = 0.093 and 0.069, respectively).

Conclusion: [18F]FDG PET/ CT shows a high NPV in penile cancer patients without clinically evident LNM. However, due to its limited sensitivity (especially with respect to LNM of small size) and specificity (i. e. in the differentiation between (post)inflammatory and metastatic lymph nodes) [18F]FDG PET/CT cannot replace invasive nodal staging.

Zusammenfassung

Ziel: Das akkurate Staging von Peniskarzinompatienten erfordert invasive Methoden wie die Sentinellymphknotenbiopsie oder die Lymphadenektomie (LAD). Wir evaluierten die Wertigkeit der [18F]FDG PET/CT beim nicht-invasiven nodalen Staging von Peniskarzinompatienten vor inguinaler LAD.

Patienten und Methodik: 41 konsekutive Patienten mit Peniskarzinom (pT1 oder höher, cN0) erhielten eine [18F]FDG PET/CT vor bilateraler modifizierter oder radikaler inguinaler Staging LAD. Lymphknoten mit einer visuell gesteigerten [18F]FDG Aufnahme wurden als metastasensuspekt gewertet. Der standardized uptake value (SUV) der suspekten inguinalen Lympknoten wurde bestimmt. Die Ergebnisse der [18F]FDG PET/ CT wurden mit der Histopathologie korreliert.

Ergebnisse: Insgesamt wurden 623 Lymphknoten reseziert, bei 10 Patienten wurden Lymphknotenmetastasen histologisch bestätigt (14/623 Lymphknoten). In der patientenbasierten Analyse zeigte die [18F]FDG PET/CT eine Sensitivität und Spezifität von 80 % bzw. 68 %, einen positiv prädiktiven Wert (PPV) von 44 % und einen negativen prädiktiven Wert (NPV) von 91 %. In der leisten-basierten Analyse hatte die [18F]FDG PET/CT eine Sensitivität von 69 %, eine Spezifität von 77 %, einen PPV von 36 % und einen NPV von 93 %. Es zeigte sich kein signifikanter Unterschied im SUVmean und SUVmax zwischen richtig-positiven und falsch-positiven Lymphknoten (p = 0.093 bzw. 0.069).

Schlussfolgerung: Die [18F]FDG PET/CT zeigt einen hohen NPV bei Peniskarzinompatienten ohne klinisch auffällige Lymphknoten. Aufgrund der limitierten Sensitivität (insbesondere im Hinblick auf die Detektion kleiner Lymphknotenmetastasen) und Spezifität (in der Differenzierung (post)inflammatorischer und metastatischer Lymphknoten) kann die [18F]FDG PET/CT das invasive nodale Staging nicht ersetzen.
