Was ist neu?

Relevanz der Detektionsmethoden Als Standard zum Nachweis von antineutrophilen zytoplasmatischen Antikörpern (ANCA) galt bis dato die indirekte Immunfluoreszenz (IIF) gefolgt von PR3- und/oder MPO-ANCA-Immunoassay. Neuere Studien haben allerdings gezeigt, dass die Verwendung von antigenspezifischen Immunoassays als einzigem Diagnoseschritt eine bessere Option darstellt.

Aktuelle Entwicklungen in der ANCA-Methodik Neben ELISA sind heute neue Festphasentechnologien (ALBIA, CLIA, FEIA, LIA/DIA) und automatisierte IIF-Assays verfügbar. Derzeit erhältliche Immunoassays für PR3- und MPO-ANCA sind hoch sensitiv und hochspezifisch für die Diagnose einer GPA und MPA.

EUVAS-Studie zur Evaluation von ANCA IIF und antigen-spezifischer Immunoassays In einer aktuellen multizentrischen Studie der European Vasculitis Study Group (EUVAS) wurde die Bedeutung der IIF-Methode im Vergleich zu antigenspezifischen Immunoassays für den ANCA-Nachweis untersucht. Die Ergebnisse bestätigen, dass die diagnostische Wertigkeit der PR3- und MPO-ANCA-Immunoassays der IIF überlegen ist. Auf Basis dieser Daten wurden jüngst neue Leitlinien zur ANCA-Testung erarbeitet.

Neue Konsensus zur ANCA-Diagnostik Die neue internationale Konsensus-Empfehlung wurde 2017 erarbeitet. Sie beinhaltet, dass PR3- und MPO-ANCA-Immunoassays für die korrekte Diagnosestellung bei AAV ohne die Notwendigkeit einer zusätzlichen IIF eingesetzt werden können.

Rationaler Einsatz der ANCA-Testung Wegen der ausgesprochenen Seltenheit der AAV spielt die Vortestwahrscheinlichkeit bei der ANCA-Diagnostik eine besonders große Rolle. ANCA sollten demnach nur bei einer mindestens mittleren Vortestwahrscheinlichkeit eingesetzt werden, d. h. wenn ein klinisch begründeter Verdacht auf das Vorliegen einer Vaskulitis besteht.

Abstract

Anti-neutrophil cytoplasmic antibodies (ANCA) play a central role in the diagnostic and pathogenesis of patients with small vessel vasculitis, so called ANCA-associated vasculitis (AAV). ANCA in these diseases are almost always directed against proteinase 3 (PR3-ANCA) and myeloperoxidase (MPO-ANCA). Most laboratories worldwide use as standard the indirect immunofluorescence technique (IFT), on human neutrophils, to screen for ANCA, and then confirm positive IFT results with antigen specific immunoassyas for PR3- and MPO-ANCA. New guidelines for ANCA testing have been developed based on a recent European multicentre study, and according to the revised 2017 international consensus recommendations, testing for ANCA in small vessel vasculitis can be done by PR3- and MPO-ANCA immunoassays, without the categorical need for IIF. The clinical utility of ANCA depends on type of assay performed and the appropiate ordering of testing the right clinical setting. Accurate identification of all patients with AAV and the avoidance of misdiagnosis can be achieved using a “gating policy” based on clinical information given to the laboratory at the time of request.
