Der diagnostische Stellenwert der Isocitrat-Dehydrogenase-Mutationen beim Chondrosarkom
Im Jahre 2008 wurden erstmalig durch Parsons et al. bei einer kleinen Gruppe von jungen Gliompatienten somatische Mutationen in dem Gen für das Enzym Isocitrat-Dehydrogenase IDH1 (Chromosom 2q33) entdeckt [1]. Wenig später folgte der Nachweis der IDH1-Mutation bei dem zentralen Chondrosarkom durch Amary und Kollegen [2].

Isocitrat-Dehydrogenasen 1 und 2 sind Enzyme des Citratzyklus (Krebs-Zyklus), die in den meisten Körperzellen vorkommen und als Katalysatoren bei der reversiblen oxidativen Decarboxylierung von Isocitrat zu Alpha-Ketoglutarat (α-KG) dienen. IDH1 und 2 stellen reduziertes Nicotinamidadenindinukleotid-phosphat (NADPH) durch den Einsatz von NADP+als Kofaktor bereit. IDH1 ist sowohl im Zytosol als auch in den Peroxisomen vorhanden, während IDH2 nur in den Mitochondrien vorkommt [3] [4]. Unter physiologischen Bedingungen stehen die Spiegel von Isocitrat und α-KG im Gleichgewicht, was den Status des zellulären Energiehaushalts reflektiert [3] [4]. Weiterhin scheinen IDH-Enzyme eine essentielle Rolle bei der zellulären Protektion und Antwort auf oxidativen und energetischen Stress zu spielen [5]. So hat das NADPH eine wichtige Rolle bei der Regeneration des antioxidanten Glutathions [6].
Der Effekt der IDH-Mutation scheint sowohl durch den Verlust der normalen katalytischen Funktion als auch durch das Erlangen einer neuen Funktion erklärbar zu sein. Die IDH-Mutation führt dazu, dass anstatt der Umwandlung von Isocitrat in α-KG eine Verstoffwechselung von α-KG in 2-Hydroxyglutarat (2-HG) erfolgt [7] [8]. Dies führt zu einer mehr als hundertfach erhöhten Konzentration des Metaboliten 2-HG in den Tumorzellen [7] [8]. Für die 2-HG-Produktion ist die Vorlage sowohl eines Wildtyp- als auch eines mutierten IDH-Enzyms notwendig, damit das intakte Enzyme α-KG produziert wird, welches dann als Substrat für die 2-HG-Bildung durch das mutierte Enzym dient [9]. Die IDH1-Mutation katalysiert die Bildung des D-Enantiomeren des 2-HG (D-2-HG), während die IDH2-Mutation die Akkumulation von R-2-HG hervorruft [10] [11]. Für beide Enantiomere von 2-HG wurde eine Rolle als Onkometabolit in chondrogenen Tumoren postuliert [12]. Dafür spricht die Beobachtung, dass das seltene angeborene Syndrom „Hydroxyglutaric aciduria“, das durch die erhöhte 2-HG-Konzentration im Urin charakterisiert ist, gehäuft mit einer Enchondromatose assoziiert ist [13] [14]. Weitere bekannte hereditäre Tumorsyndrome, die mit Enchondromatosen auf dem Boden von IDH1/2-Keimbahnmutationen einhergehen, sind die Ollier Krankheit und das Maffuci Syndrom [15] [16].
Bei allen IDH-Mutationen handelt es sich um eine Punktmutation, bei der die Aminosäure Arginin in dem Kodon (Basentriplett) 132 bei der IDH1-Mutation und in dem Kodon 172 bei der IDH2-Mutation durch eine andere Aminosäure ersetzt wird [1] [17]. Die häufigsten Varianten der IDH1/2-Mutationen bei chondrogenen Tumoren sind IDH1 R132C, R132L, R132G und IDH2 R172S, R172G, R172M [2]. Weitere häufige Alterationen, die bei Chondrosarkomen beschrieben wurden, sind TP53- Mutationen und CDKN2A/B-Deletionen [18].
Die Häufigkeit der IDH-Mutation variiert bei chondrogenen Tumoren in Abhängigkeit von ihrer Histologie. So scheint die IDH-Mutation spezifisch für Enchondrome und Chondrosarkome zu sein [18]. Insgesamt lassen sich bei etwa 50% der WHO Grad 2 und 3 Chondrosarkome und in bis zu 87% der dedifferenzierten Chondrosarkome IDH-Mutationen nachweisen [18]. Dagegen weisen klarzellige und mesenchymale Chondrosarkome keine IDH-Mutationen auf. Das Implementieren der IDH-Mutation in der 5. Auflage der WHO-Klassifikation für Weichteile- und Knochentumore deutet auf ihre Relevanz in der histologischen Diagnostik bei chondrogenen Tumoren (ISBN 978-92-832-4502-5). Des Weiteren stellt die Erfassung der IDH-Mutation die Grundlage für die Einteilung der Chondrosarkome in zwei „molekularen“ Gruppen. Diese könnte der Risikostratifizierung dienen und eventuell die prognostische Genauigkeit der histologiebasierten Einteilung der Chondrosarkome wiederspiegeln [19]. So scheinen vor allem Patienten mit IDH-mutierten Chondrosarkomen WHO Grad 2 und 3 eine günstigste Prognose in Bezug auf das rezidivfreie Überleben und auf die Entwicklung von Fernmetastasen im Vergleich zu Wildtyp-Patienten zu haben [19].
Die oben beschriebene spezifische Akkumulation von 2-HG in den Tumorzellen könnte für die präoperative nichtinvasive Bestimmung des Mutationsstaus mittels Magnet-Resonanz-Spektroskopie (MRS) genutzte werden. Die 2-HG MTS ist bis dato lediglich bei Gliomen etabliert mit hoher Sensitivität und Spezifität [20]. Problematisch ist allerdings, dass mit dieser Methode keine Feststellung des Tumorgrads möglich ist. Aus diesem Grund ist üblicherweise eine operative Gewebeentnahme für histopathologische und molekulare Untersuchungen unumgänglich.
Die IDH-Mutation stellt ein häufiges genomisches Ereignis bei Enchondromen und Chondrosarkomen dar. Ihre Relevanz in der histopathologischen Diagnostik führte zu Ihrem Implementieren in der aktuellen WHO-Klassifikation. Die Evaluation der bildmorphologischen und therapeutischen Bedeutung der IDH-Mutation bei chondrogenen Tumoren ist Gegenstand aktueller Forschung.
