Peroral endoscopic tunneling transgastric drainage (POET-D) is an advanced endoscopy technique that provides drainage of fluids in complex, often high risk clinical settings. The case report and video by Manolakis et al. provides an example of this cutting-edge technique. The video highlights in detail the technical aspects of the procedure. Following the principles of natural orifice transluminal endoscopic surgery (NOTES), the technique is based on submucosal tunneling with the use of a mucosal flap.

While POET-D remains an interesting and promising technique, its clinical application continues to be defined. In this case, a fluid collection adjacent to the stomach could not be drained by proven techniques such as endoscopic ultrasound (EUS)-guided or percutaneous drainage. This is an uncommon scenario, and POET-D offered an alternative to the much more invasive option of a traditional surgical approach.
