Einleitung

Teil 2 beschreibt das akute Skrotum, das durch entzündliche Erkrankungen und seltene Krankheitsbilder hervorgerufen wird. Die Diagnose basiert auf der klinischen Untersuchung, der Sonografie (US) und dem Farbdoppler-US. Nach neueren Studien zeigte der kontrastverstärkte US (CEUS) eine höhere Genauigkeit als der Farbdoppler, wenn es darum geht, den Blutfluss in pathologischen Gefäßen zu bewerten. Entzündliche Erkrankungen des Skrotums sind durch eine Hyperämie gekennzeichnet und können vor allem mittels Graustufen-US und Farbdoppler-US untersucht werden. In seltenen Fällen kann insbesondere CEUS zur Diagnose von Komplikationen hilfreich sein. Konventionelle Radiologie, Computertomografie (CT) und Magnetresonanztomografie (MRT) werden für gewöhnlich nicht eingesetzt, diese Methoden können aber teilweise von Nutzen sein, um Gasblasen nachzuweisen. Im Folgenden werden einige seltene Erkrankungen im B-Bild- und Farbdoppler-US beschrieben. Ergänzt wurde der Leistenbruch, da die Symptome dieses häufigen Syndroms dem Erkrankungsbild des akuten Skrotums ähneln können. Die Differenzialdiagnostik ist von Bedeutung, da die jeweilige Behandlung unterschiedlich ist.
