Background: The aim of this study was to represent the outcomes of purely endoscopic transnasal resection for sinonasal malignant tumor with intracranial extension.

Methods: A retrospective review was performed on the patients who underwent purely endoscopic endonasal resection of anterior skull base tumor in a single institution between April 2015 and June 2018. Demographics, histology, surgical management, surgical outcomes, complications, and morbidity were analyzed. Prognostic factors were evaluated with a multivariate analysis.

Results: A total of 58 patients underwent purely endoscopic endonasal resection of anterior skull base tumor, of whom 17 patients were with olfactory neuroblastoma, 9 patients with squamous carcinoma, 8 patients with meningoma, and 8 patients with melanoma and sarcoma. The mean age was 51 years (ranging from 20 to 78). The overall gross-total tumor resection rate was 85%. There was no CSF leak after surgery. There was no severe complication except ICA rupture occurred in one patient in surgery, and the patient was immediately underwent DSA. The mean duration of follow-up was 18 months (2–48 months). During our follow-up, 10 patients recurred, including 4 olfactory neuroblastomas, 3 squamous carcinomas, 3 sarcomas.

Conclusion: Endoscopic endonasal resection is an alternative approach for sinonasal malignant tumor, with less complication and exciting results.
