Introduction The submental island flap has the advantages of being easy and quick to harvest, and pliable enough to reconstruct defects of the oral cavity. Concerns exist regarding its oncological safety.

Materials and methods We report a case of verrucous cancer of the buccal mucosa where the defect was reconstructed with the use of the submental flap. The histological variant of the tumor served as an indication for the use of the flap. The Stensen’s duct was preserved and reimplanted after obtaining safe margins.

Results The wound healed uneventfully with no signs of contracture or salivary flow obstruction.

Conclusion The submental island flap is an excellent alternative to free flap for the reconstruction of moderate sized defects of the oral cavity regarding that the oncological requirement are met.
