Abstract Take-home naloxone is an evidence-based response to opioid overdose with variable implementation across the world. Drawing on work conducted in Australia, Central Asia and Ukraine this presentation will highlight the challenges to program implementation including determining target populations and navigating regulatory hesitancy. The importance of contexts for models of service delivery and establishing targets and impacts will be discussed. Resources for the rapid upscaling of take-home programs will be presented along with the importance of consumer representation.
