A 66-year-old female patient with a large left clinoid meningioma wrapping around
the trunk and bifurcation of the internal carotid artery, elevating the left middle
cerebral artery, and invading the left optic canal. We demonstrate the safe performance
of a cranio-orbital craniotomy with extradural anterior clinoidectomy, optical unroofing,
and piecemeal tumor removal to cautiously unwrap the tumor from the encased left ICA
trunk, branches, and bifurcation for gross total resection.
