Tumors of the petrous apex are particularly challenging because of the complex anatomy
in this region. We are describing a novel technique on approaching contralateral petrous
apex and clival lesions with preservation of the nasal structures using a mononostril
technique through the contralateral sphenoid sinus. This is achieved by using the
chopsticks technique in endoscopy along with angled endoscopes allowing a unique corridor
into the lateral compartment of the petrous apex.
