Abstract Text A 74-year-old woman showed a circumferential post-ESD scarring stenosis of the rectum
(10 cm from the anal verge), which recurs despite several pneumatic dilations and
placement of fully covered metal stent.
We performed endoscopic circumferential resection of the scar with ESD knives, achieving
a significant widening of the stenosis, without need for balloon dilatation [1]

We apposed two TTS clips for suspected muscle fiber slimming and triamcinolone was
locally injected at the end, in order to avoid stenosis recurrence.
No adverse events occurred.
Endoscopic follow-up one month after treatment showed widening of the substenosis.
Clinical 3-months follow-up confirmed relief of symptoms.
