Abstract Text The authors present two gastrointestinal stromal tumors, one located in the fundus
(5 cm) and the other in the gastric body (3 cm), removed by endoscopic full-thickness
resection. CT scan and endoscopic ultrasound showed fourth layer lesions without local
invasion. A needle-tip (DualKnife J, Olympus, Japan; Bipolar knife, AGS, China) and
insulated-tip (IT-nano, Olympus) knifes were used for dissection. Exposure of muscularis
propria attachment and deeper plane dissection was facilitated by a pulley traction
with elastic band, fishing-line and TTS clips (Creo Medical, UK). The defects, showing
peritoneal fat and the liver, were closed using two side-by-side OTS-clips (Ovesco,
Germany). There were no complications. Pulley traction and OSTC clips are useful tools
for faster and safer full-thickness endoscopic resection of subepithelial lesions,
including larger lesions in difficult locations.
