Abstract Text The finding of a dilatation of the main pancreatic duct (MPD) is relatively common,
representing a differential diagnosis between benignity and malignancy. Suspecting
malignancy often leads to consideration of a pancreatectomy, which, although it can
be performed safely, carries high morbidity (50-60%) and a mortality rate of 5%. Therefore,
it is crucial to accurately establish the diagnosis. We present two cases of MPD dilatation.
Spyglass pancreatoscopy was performed and determined the indication for pancreatectomy.
The use of pancreatoscopy in the diagnostic arsenal for MPD dilatation with suspected
IPMN is highly valuable. Targeted biopsy has low sensitivity, but visual impression
provides high specificity in confirming the diagnosis [1]
