Die Kraftausübung bei der Gaumennahterweiterung nach Derichsweiler

Zusammenfassung

Zur Messung der bei der Gaumennahterweiterung entstehenden Kräfte wurde ein spezielles Messverfahren entwickelt. Mithilfe dieses Messverfahrens wurde bei neun Patienten eine Gaumennahterweiterung nach Derichsweiler durchgeführt. Die auf den Oberkiefer wirkenden Kräfte stiegen auf bis zu 66 N an. Das Kraftniveau schwankte von Patient zu Patient deutlich. Nach erfolgter Eröffnung der Gaumennaht fielen bei einem Patienten die Kräfte ab. Es blieb eine Restspannung übrig, die vermutlich durch die noch bestehenden Spannungen in Weichteilen und Knochen zu begründen ist. Ebenso sind nicht vollständige Frakturen denkbar, da bei den anderen Gruppen kein Kraftabfall nach der GNE zu verzeichnen war. Die über 3-4 Wochen aktivierten Schrauben erzeugten kaum Beschwerden für die Patienten. Bei den meisten Patienten stiegen die Kräfte bei der Aktivierung nach Derichsweiler nur auf ca. 20-25 N an, ohne später abzufallen. Ein Herausbewegen der Zähne aus dem Knochen oder eine Fenstration bei den Molaren kann damit nicht ausgeschlossen werden.

Abstract

To measure the forces occurring during rapid maxillary expansion, a special measuring method was developed. With the help of this measuring method rapid maxillary expansion according to Derichsweiler was carried out in nine patients. The forces acting on the maxilla increased up to 66 N. The force levels varied considerably from patient to patient. After the “rupture” of the midpalatal suture the forces decreased in one patient. The remaining forces seemed to be the result of stress in soft tissue and bone. Incomplete fractures are also imaginable, since no force decreases occurred after the RME in the other groups. The screws activated for 3 to 4 weeks hardly caused any discomfort for the patients. In most of the patients the forces increased only to about 20 to 25 N during activation according to Derichsweiler and did not decrease later on. Consequently, a buccal movement of teeth or a fenestration of the molars cannot be excluded.
