Klinische Beurteilung von Kronenpassungen - Einfluss des marginalen Randspaltes und der Stufenkontur

Entscheidende Kriterien für den Erfolg einer Kronenrestauration sind die Größe des marginalen Randspaltes und die Kontur des Kronenrandes. Die Bestimmung dieser Parameter ist im klinischen Alltag allerdings nur nach subjektivem Ermessen möglich. Diese Studie beschreibt die Korrelation zwischen der objektiven marginalen Randspaltbreite und der Kronenkontur und der subjektiven Einschätzung der Kronenpassung durch Zahnärzte und Zahntechniker. Bei Vollgusskronen aus verschiedenen Legierungen und Herstellungstechniken (Gusstechnik: AuAgCu, AuPdPt, PdAgAu, CoCrMo, Ti und Frästechnik: Ti) fanden sich sowohl marginale Randspalten als auch überkonturierte Kronenränder. Die gemessenen Parameter korrelierten signifikant (p < 0,05) mit der subjektiven Einschätzung der Kronenpassung durch Zahnärzte und Zahntechniker. Sowohl die Entscheidung, ob der marginale Randschluss akzeptabel ist, als auch, ob die Krone klinisch zementiert werden kann, wurde nur vom Ausmaß der Überkonturierung signifikant beeinflusst.

Important criterions for the success of a crown are the size of its marginal gap and the size of its possibly overextended margin. However, in the patient's mouth, its fit can only be evaluated by subjective measuring. This study describes the correlation between the objective marginal gap and overextended margin of crowns and the subjective evaluation of their fit by dentists and dental technicians. Crowns from different alloys and techniques (casting-technic: AuAgCu, AuPdPt, PdAgAu, CoCrMo, Ti, and mill-technic: Ti) showed differences of the marginal gap and of the overextended margin. The objective marginal gap and the overextended margin of the crowns significantly (p < 0.05) correlated with the subjective evaluation of their fit by dentists and dental technicians. For the decision of the clinicians, if the marginal fit is clinically acceptable only the overextended margin had a significant influence.
