Zusammenfassung

Passive Mobilisation wirkt allgemein schmerzlindernd und kann daher auch bei schmerzhaften Bewegungseinschränkungen der Frozen Shoulder effektiv sein. Die schwache bis moderate Evidenz in der Literatur ist diesbezüglich unklar. Bei der im Krankheitsverlauf der Frozen Shoulder entstehenden bindegewebigen Gelenkkontraktur ist die manuelle Mobilisation unwirksam.

Der vorliegende Beitrag erläutert anhand einer narrativen Literaturanalyse die Ursachen einer bindegewebigen Gelenkkontraktur und stellt Behandlungsmöglichkeiten vor. Die tägliche stundenlange Behandlungsdauer scheint hierbei eine entscheidende Rolle zu spielen. Einige Studien erreichten dies mit der Schienenbehandlung. Die praktikable Alternative der Lagerung wird als Teil eines Behandlungsprogramms bei bindegewebigen Gelenkkontrakturen vorgestellt.

Abstract

Passive mobilisation shows general pain-relieving effects and can therefore also be effective for movements limited by pain in frozen shoulder. The poor to moderate evidence in the literature is however unclear in this regard. The connective tissue joint contracture occurring during the progression of frozen shoulder does not respond to manual mobilisation effectively.

This article explains the sources of a connective tissue joint contracture on the basis of a narrative literature review and describes possible treatment modalities. Daily treatment lasting over a period of hours seems to play a vital role. Splints have been successfully implemented in several studies. The feasible alternative of positioning as part of a treatment programme for connective tissue joint contractures is described.
