Zusammenfassung

Die Arthrosonografie hat aufgrund ihrer hohen Sensitivität im Aufdecken entzündlicher Weichteil- und Knochenveränderungen einen bedeutenden Stellenwert in der Früharthritisdiagnostik erlangt. Die Gelenkultraschall-Untersuchung im Brightscan-Bild kann, zusätzlich zu den klinisch und laborchemisch erhobenen Daten, Hinweise auf das Vorliegen eines entzündlich-rheumatischen Gelenkprozesses geben, und der Farb-/Power Doppler-Ultraschall macht eine Differenzierung von aktiven und nicht-aktiven entzündlichen Veränderungen im Bereich von Gelenken und Sehnen möglich. Somit hat das Ultraschallverfahren an den peripheren Gelenken und an periartikulären Strukturen diagnostische als auch differenzialdiagnostische Bedeutung. Dieser Artikel beschäftigt sich mit typischen sonografischen Befunden rheumatologischer Gelenkerkrankungen, auch im Vergleich zu anderen bildgebenden Verfahren, und geht außerdem auf die neuesten wissenschaftlichen Entwicklungen in der Gelenksonografie ein.

Abstract

Musculoskeletal ultrasonography has a high sensitivity in detecting inflammatory soft tissue, periarticular changes and early bone lesions. Therefore it has a great impact on the early diagnosis of rheumatic diseases. Ultrasonography in B-mode is helpful in confirming the diagnosis of assumed joint diseases, especially in combination with clinical and laboratory data. The colour/power Doppler mode is able to differentiate between active and non-active (peri-)articular inflammatory processes. Consequently, musculoskeletal ultrasonography is an important tool in the diagnosis and differential diagnosis of arthritic diseases. This article illustrates the typical ultrasonographic findings in different rheumatic diseases, also in comparison to other imaging methods, and it furthermore summarises recently evaluated clinical research results in musculoskeletal ultrasonography.
