Präklinische Multiple Sklerose und Therapieoptionen

Zusammenfassung

Durch den breiten Einsatz der Schnittbildgebung häufen sich radiologische Zufallsbefunde. Der vorliegende Fall beschreibt eine Patientin mit kernspintomografisch nachgewiesener Dissemination der Entzündung in Ort und Zeit ohne fassbares klinisches Korrelat (sog. präklinische Multiple Sklerose [MS]), bei der wir uns zu einer Frühbehandlung mit Interferon β-1b entschieden. Die spezifischen Probleme dieses Konzepts bezüglich Diagnosestellung, Patientenaufklärung und Therapieeinleitung werden anhand der Kasuistik sowie der aktuellen Literatur diskutiert.

Abstract

Due to the broad use of magnetic resonance imaging (MRI) incidental radiological findings are becoming more frequent. The present case describes a female patient with dissemination of inflammation in time and space confirmed by MRI without evidence of a clinical correlate (referred to as preclinical multiple sclerosis [MS]) which we decided to manage with early treatment with Interferon β-1b. The case history and the current literature form the basis for the discussion of the specific problems associated with this concept regarding diagnosis, patient information and initiation of therapy.
