Fallbericht über ein spinales Tuberkulom

Zusammenfassung

Die Inzidenz der spinalen intraduralen extramedullären Tuberkulose ist gering und wird dank der medizinischen Vorsorge immer seltener. Wir berichten über einen Fall einer 55jährigen Patientin, die eine akute spastische Paraparese der Beine entwickelte. Die Diagnostik mittels Myelogramm und Computertomographie der thorakalen Wirbelsäule zeigte eine Raumforderung von Th 2 bis Th 6, die sich bei der neurochirurgischen Operation als Tuberkulom der Dura herausstellte.

Der Fall ist wegen der Lokalisation und der Histologie und aufgrund des Fehlens eines tuberkulösen Primärherdes selten.

Summary

The incidence of intradural extramedullary tuberculomas of the spinal cord is rare and is getting still rarer as medical care improves. We report on a case of a 55 year old woman who developed acute spastic paraparesis of the legs. Investigation including myelography and computerised tomography of the thoracal spine revealed a large lesion from T2 level to T6 which proved to be a tuberculoma of the dura by neurosurgical operation. The case is rare in size, location, and nature of the lesion and the absence of the tuberculous primary foci.

The authors advice is to perform surgery followed by antituberculosis treatment.
