    Background Few reports of POEM as treatment for achalasia in Allgrove syndrome has been reported.A 27 year old woman with Allgrove syndrome. Surgical and medical approaches failed thus endoscopic approach was decided. High-resolution esophageal manometry-impedance (HREM-I) showed type I achalasia and severe bolus retention. Endoscipic findings: POEM performed using a T type hybrid knife with no complications. HREM-I one month after the procedure showed complete relaxation of the inferior sphincter and improvement on the bolus retention. No dysphagia or reflux on the follow-up.

    Conclusions POEM could be an effective treatment in Allgrove syndrome related Achalasia.
