In diesem Beitrag werden die wichtigsten Erkrankungen skizziert, die ätiologisch einer Small-Fiber-Neuropathie (SFN) zugrunde liegen können. Schwerpunkt des Artikels bildet die Charakterisierung der im klinischen Alltag bereits etablierten sowie der in der aktuellen klinischen Forschung untersuchten Verfahren zur Diagnostik einer kutanen autonomen Dysfunktion.

Kernaussagen
  • Small-Fiber-Neuropathien (SFN) betreffen nicht myelinisierte C-Fasern und dünn myelinisierte Aδ-Fasern.

  • Eine Vielzahl an systemischen und hereditären Erkrankungen kann pathogenetische Grundlagen bilden.

  • Da diese Fasern u. a. an der Vermittlung von Schmerz- und Temperaturempfinden beteiligt sind, imponiert bei Schädigung sensorisch häufig ein neuropathisches Schmerzsyndrom (quälende Dys- und Parästhesien).

  • Im Falle einer autonomen Small-Fiber-Neuropathie kommt es zu zusätzlichen autonomen Störungen (z. B. Schweißsekretionsstörungen, vasomotorische Störungen, gastrointestinale Symptome, erektile Dysfunktion), da diese Nervenfasern zudem maßgeblich an der parasympathischen und sympathischen Steuerung der Endorgane beteiligt sind.

  • Motorik, Propriozeption und Elektrophysiologie zeigen sich in der klinischen Untersuchung meist intakt.

  • In der Hautbiopsie zeigt sich die intraepidermale Nervenfaserdichte in der Regel reduziert.

  • Weitergehende Diagnostik ist derzeit auf spezialisierte Zentren beschränkt.

  • Zur Testung auf vorliegende Einschränkung der betroffenen Faserqualitäten kommen vasomotorische (LDI/LDF), sudomotorische (QSART) und pilomotorische (QPART) Testverfahren zum Einsatz, denen eine Stimulation des Axonreflexes durch Iontophorese geeigneter medikamentöser Stimulanzien zugrunde liegt.

  • Therapeutisch spielt die ätiologische Zuordnung der diagnostizierten SFN eine entscheidene Rolle, um möglichst eine kausale Therapie einleiten zu können, was den Langzeitverlauf der Erkrankung entscheidend beeinflussen kann.
