Zusammenfassung

Die Verwendung von kardialen Ersatz- und Unterstützungsverfahren ist heute klinischer Alltag. Anästhesisten sollten mit diesen Systemen gut vertraut sein – denn sowohl bei deren Implantation als auch bei der Betreuung der betroffenen Patienten spielt der Anästhesist eine wichtige Rolle. Der Beitrag erläutert die Funktionsweise, die Indikationen und die Therapieziele der wichtigsten Herzersatz- und -unterstützungssysteme.

Abstract

The management of acute and chronic heart failure utilizes various different techniques and medical devices. Different mechanical chest compression devices (mCPR), intraaortic balloon pump, catheter-based heart pumps like Impella®, and different kinds of ECLS/ECMO are mainly used as bridge to recovery. Venovenous ECMO gained wide acceptance after the CESAR trial. Venoarterial ECMO plays an increasing role as extracorporeal CPR (eCPR). Newer concepts include venovenoarterial ECMO and the combination of ECMO and Impella. Principles and limitations of these concepts are discussed. Surgically implanted devices that include left and right ventricular assist devices (LVAD, RVAD), biventricular VADs and the total artificial heart (TAH) are commonly used as bridge to transplant or destination therapies. Quality of life must be considered in the use of the devices, as the life-saving devices come with restrictions in the patientsʼ lives.

  • Die Verwendung von kardialen Ersatz- und Unterstützungsverfahren ist klinischer Alltag. Dennoch ist ein großes Maß an Expertise erforderlich, um dem Patienten das bestmögliche Behandlungsergebnis zu ermöglichen.

  • Mechanische Thoraxkompression (mCPR) kann trotz fehlendem Nachweis für ein besseres Überleben sinnvoll sein, um die Zeit bis zu einer Intervention zu überbrücken.

  • Die intraaortale Ballonpumpe (IABP) hat an Bedeutung verloren, in Einzelfällen wird sie bei kardiochirurgischen Patienten eingesetzt.

  • Die katheterbasierten Pumpen erlauben eine wenig invasive Unterstützung sowohl des linken als auch rechten Ventrikels. In ihrer Anwendung können sie mit ECMO- oder VAD-Systemen kombiniert werden.

  • Nach dem CESAR-Trial hat die Anwendung von ECMO, sowohl venovenös als auch venoarteriell, deutlich zugenommen. Für die Anwendung zur eCPR gibt es aktuelle deutsche Konsensusempfehlungen.

  • LVAD-Systeme können sowohl als Überbrückung bis zu einer Transplantation als auch als Therapie bis zum Lebensende eingesetzt werden.

  • BiVAD und TAH bleiben Optionen für Patienten mit biventrikulärem Versagen, die auf eine Herztransplantation warten. Die mögliche Anwendung des TAH als „Destination Therapy“ wird derzeit untersucht.
