Zusammenfassung

Hintergrund Im Rahmen der antegraden Humerusmarknagelosteosynthese können zugangsbedingte Läsionen der Supraspinatussehne mit einer postoperativen Funktionseinschränkung der Rotatorenmanschette (RM), Insuffizienzen oder Adhäsionen einhergehen und dadurch zu chronischen Schulterschmerzen führen. Diesbezüglich besteht jedoch eine überschaubare Datenlage bzw. Evidenz.

Patienten/Material und Methoden Anhand eines Fallbeispiels beschreiben wir erstmals bei einer dislozierten diaphysären Humerusfraktur einer geriatrischen Patientin einen die RM aussparenden ossären Zugang, ohne die vulnerable Supraspinatussehne (SSP) zu schädigen. Dies ist mittels einer begrenzten Osteotomie des Tuberculum majus durch Präparation eines ca. 10 mm tiefen und 15 – 20 mm breiten knöchernen SSP-Ansatzes möglich. Der Markraum wird zum Einbringen des Nagels eröffnet und nach Stabilisierung der Fraktur wieder verschlossen durch Reposition der hierdurch angelegten Tuberculum-majus-Osteotomie, bspw. mit einem Fadenanker. Das Vorgehen beinhaltet auch eine Bizepssehnentenodese. Neben der Darstellung des intraoperativen Vorgehens wurden die Ergebnisse klinisch-funktionell anhand etablierter Scores sowie radiologisch und sonografisch 6 Wochen (T1) und 3 Monate (T2) postoperativ erfasst.

Ergebnisse Die Patientin zeigte sich hinsichtlich Schmerzen und Funktion mit dem OP-Ergebnis sehr zufrieden. Die RM-Tests waren negativ und es bestanden keine Impingement-ähnlichen Symptome oder Hinweise für eine adhäsive Kapsulitis. Die Bewegungsausmaße, der Constant- sowie (Quick)DASH-Score zeigten im Verlauf eine sehr gute Funktion. Radiologisch zeigte sich eine regelrechte Implantatlage und gute knöcherne Konsolidierung. Die sonografische Kontrolle des linken Schultergelenkes stellte ein reizloses Gelenk dar ohne Erguss bzw. periartikuläre Flüssigkeit und keine Adhäsionen mit der Deltafaszie. Die RM, insbesondere die SSP, zeigte eine normale sonografische Abgrenzbarkeit. Die funktionellen Ergebnisse sind mit den bisherigen Publikationen vergleichbar.

Schlussfolgerung Die hier beschriebene OP-Technik zeigt einen deutlich weniger invasiven Zugang zur Implantation eines antegraden Humerusmarknagels mit einer freien Schultergelenkfunktion im kurzfristigen Nachuntersuchungszeitraum. Dieser neuartige Zugang sollte zukünftig durch prospektive Studien hinsichtlich des funktionellen Outcomes untersucht werden, um die dauerhafte Etablierung des Verfahrens begründen zu können. Andere Zugangsmodifikationen wurden beschrieben, unterscheiden sich jedoch essenziell von dem hier beschriebenen Vorgehen hinsichtlich der Invasivität der vulnerablen tendinösen Struktur der SSP bzw. RM.
