Zusammenfassung
Die Psoriasis vulgaris stellt eine häufige, chronisch entzündliche Hauterkrankung
dar. Bis zu einem Drittel der Betroffenen leiden zusätzlich unter Psoriasis-Arthritis,
einhergehend mit schmerzhaften Gelenkentzündungen, die unbehandelt zur Gelenksdestruktion
führen können [1]. Trotz eines enormen Fortschritts in den Möglichkeiten der Systemtherapie stellen
schwere Verläufe, insbesondere in der Schwangerschaft, immer noch eine Herausforderung
dar. Wir berichten über den Fall einer 32-jährigen schwangeren Patientin mit seit
dem Jugendalter bestehender, schwerer Psoriasis vulgaris et arthropathica. Unter kombinierter
Lokal-, UV- sowie Systemtherapie mit Certolizumab pegol (Cimzia®) konnte eine stabile Krankheitskontrolle erreicht und in der 40. Schwangerschaftswoche
ein gesundes Mädchen entbunden werden.
Abstract
Psoriasis vulgaris is a common chronic inflammatory skin disease. Up to one third
of these patients also suffer from psoriasisarthritis with painful inflammations of
the joints. This may lead to a destruction of the involved joints without therapy.
Despite the availability of several new systemic therapies, the management of severe
psoriasis in pregnancy still is challenging. We report the case of a 32-year-old pregnant
woman with a severe psoriasis vulgaris with arthritis since childhood. Under combined
local-, ultraviolet and systemic therapy with certolizumab pegol we gained a stable
disease control. Our patient gave birth to a healthy daughter in the fortieth week
of pregnancy.
