Zusammenfassung

Die für eine aus Sicht von Patienten, Arzt und Krankenhausträger erfolgreiche stationäre Behandlung erforderlichen Schritte sind vielschichtig und müssen sinnvoll miteinander verzahnt werden. Dies trifft in besonderem Maße für operativ tätige Fächer zu. Zur Koordination dieser Schritte, die mit der Planung der stationären Aufnahme beginnen und der Weiterversorgung nach Entlassung enden, bietet sich das Konzept des Case Managements an.
