Zusammenfassung

In den letzten Jahren haben die Erkenntnisse über die Zusammensetzung des humanen Mikrobioms mit seinen vielfältigen Interaktionen und Stoffwechselleistungen zunehmend auch Einfluss auf praxisrelevante Therapieentscheidungen zur Folge. Das trifft in ganz besonderem Maße für den Bereich der Onkologie zu [2] [8] [9]. Neben operativen und strahlentherapeutischen Eingriffen bewirkt besonders der durch zahlreiche Innovationen gekennzeichnete Bereich der medikamentösen Tumortherapie oft ungewollte, aber nicht selten gravierende, und nicht auf den ersten diagnostischen Blick erkennbare negative Einflüsse auf das gesamte Mikrobiom. Unerwünschte Wirkungen hat auch die begleitende Supportivtherapie, die bei ohnehin häufig immunsupprimierten Patienten sehr intensiv Antibiotika und andere auf die Darmbesiedelung Einfluss nehmende Medikamente einsetzt. Vor diesem Hintergrund wird in dieser Fallbeschreibung der zytostatische Behandlungsverlauf einer Patientin mit Endometriumkarzinom in einer adjuvanten Therapiesituation beschrieben. Nach Abschluss der zytotoxischen Behandlung erfolgten eine Mikrobiomanalyse sowie eine daran adapierte Rekonstitution des alterierten Mikrobioms.

Abstract

In recent years, knowledge about the composition of the human microbiome with its diverse interactions and metabolic activities has increasingly influenced therapeutic decisions. This applies especially to the field of oncology. In addition to surgical and radiotherapeutic interventions, the drug-based tumor therapy often causes unwanted, but serious negative effects on the entire microbiome that are not discernible at first glance. Numerous innovations in this field did not yet solve this problem. Also, the accompanying supportive therapy, which uses antibiotics and other drugs that affect the intestine colonization to a large extent, causes adverse events in immunosuppressed patients. In this case description the cytostatic course of treatment of a patient with endometrial carcinoma in an adjuvant therapeutic situation is described. After the cytotoxic treatment had been accomplished, a microbiome analysis and an adapted reconstitution of the altered microbiome were carried out.
